I'd like to see a Survival horror sandbox game set on an island with afew small towns on it.
You play as a custom made character who is returning to his/her home town after being gone for afew years and on the outside everything seems normal, the town you arrive at is full of life. You stay in a hotel overnight (because its getting late and the main characters parents house is the otherside of the island in one of the other towns) When you finally wake up you discover that the entire town has become lifeless but it becomes clear that you may not be as alone as you may have originally thought. There are creature roaming about the island and its not safe to stay out in the open for too long.
The game would use a realtime clock that records what day it is etc. Things like time of day, day of the week and weather will effect gameplay and what kind of monstars are about, for example...
On a sunny day you may get monstars that are capable of using the the sun to there advantage where as on a rainy day you may get something that uses water to camoflage itself to sneak up on you.

But the problem with the sandbox genre is that it all sounds really cool in theory but in practise it feels a bit gimicky (See: Burnout paradise).
The Zombie/Sandbox would work i feel thoug.
and i think prototype/infamous were onto a winner but i don't like the supernatural feel.
My perfect sandbox game would be post-apocolyptic world with exellent (cinematic) story and a hint of horror - I think i just described fallout 3... but less RPG-ish. I think Episodic content would help as well

just brainstorming~
ok alot of these ideas come from a continuing story and countless idea session betwenn friends how about a multiple timeline sand box like a huge chronotrigger where u may have to go back in time to get certain items that can only be found in the past like documents metals bones etc same with traveling to the future. a big multiplayer mode has to be tied into the game as well via PVP like past wars and future wars etc. along with multiplayer freeroam kinda like White knight cronicles also should have a split screen mode too. it should have MMORPG Shooter feel to it like Fallout3 all in all it should have a Fallout 3 feel and look to it but with out the nuclear fallout maybe more of a greener world... the game should be online and offline like Phantasy Star was. theres has to be something like a customizable home dwelling or dwellings like in Runes of Magic Online or Sims. there should be alot of minigames and side quests as well as full character custimizations face, body size, facial hair, head hair, eyes, etc. there should also be alot of wardrobe options clothing armor random pieces of fabric u find and pick up can be added to your outfit as well maybe thru a designer section of the game. you should be able to loot NPCs take there clothing and anything they have on them like in oblivion pickpockets reverse pickpockets to blackmail or assassinate NPC as for players u can loot certain items off them and certain weapons as well. there needs to be transportation like vehicles horses planes boats etc. camera toggles and full control of camera as well is a plus. character jobs rather then class anyone can be anything is a cool concept u play the way you want to play and pick jobs you want to do... maybe a multidemention sort of free roam feel as well climb jumb tunnel etc.... maybe have a LittleBigPlanet/HOME asset to the game as in give players there own town there own house maybe cities but instead they have to work hard for the land for towns and cities but to rent a place is free unless you have more places kinda like home your first place is free the rest you pay for via a lot of in-game currency or a couple dollors real money or countless hours of hard work
so to sum it up it should be a typical MMO/RPG/Shooter with a Sims,Fallout 3,Oblivion,spiderman,call of duty,WoW,White Knight Cronicals,tombraider,assassins creed 2, GTA4 and Playstation HOME feel to the game with time travel as its main focus along with multiplayer online and offline action cuz sandbox games tend to get boring after time especially when your playing alone sure you have NPCs everywhere but they are limited and soul-less plus with the option to time travel gives you more options when you get bored say your on a boring killing rampage in modern time well you can go back in time and start killing cavemen or travel to the future and kill aliens maybe instead of time travel have it be different realms like in dotHACK or different planets or better yet... keep both time travel and other planets and realms or just realms could work fine too maybe constant PSN realm updates as in buy new ones earn new ones there needs to be a story too one that you can keep going back to keep adding expantions etc...
