I was playing GTA IV online last and wanted to play with someone on my friends list.

The problem was though, that although his name was there on the XMB (and he was shown to be playing GTA IV) it wasn't appearing within the game (on the phone). It seemed to have everyone else listed except him.

He told me that he was able to send me an invite, but I never received any.

What do you think is up here? Is there some sort of privacy setting he may have unknowingly activated?

I can never get this to work with certain friends on my list either, Its a bug.

When GTA IV released, the Max amount of friends you could have was 50 - But when PSN updated it to 100 Friends, GTA didn't.

GTA's online friend list only shows the first 50 friends you added - if you want to play with someone else, you'll have to delete some friends

The deleting friends fix is no longer valid Rockstar increased it to 100 in patch 1.06 along with DLC preperation and the Bomb da Base error fix. 

Best way to fix invite issues is put your PS3 in your routers DMZ your friends may need to do the same.
