Far too many games for there to be any possible answer but I'd say my favourites (so far, not ever. I don't know what the future holds) have been:

Ico
Shadow of the Colossus
Metal Gear Solid 3
Metal Gear Solid 2
Metal Gear Solid
Mass Effect
Demon's Souls
Ratchet and Clank 3
Halo
The Orange Box
Ninja Gaiden 2 (I've not played Balck but I love this)
Tekken 3
Halo 2

And there's more but I don't feel like typing out a long list. However, most of these wouldn't qualify as "best games ever" by a long shot. A more neutral list (a good few games I've not played):

Chrono Trigger
Mother 3
Ocarina of Time
Majora's Mask
Super Metroid
Baldur's Gate 2
System Shock 2
Morrowind (I think it's good, but I don't think it's this good really)
Mario 64
Rome: Total War
Final Fantasy 7 (I find it horrifically over-rated but whatever)

Soul Calibur
Civilization 4

Playstation: Final Fantasy 9

PS2: GTA Vice City

Dreamcast: Shenmue

Gamecube: Metroid Prime

Zelda: Ocarina Of Time-The amount of time i just spent fishing in that game was unreal,some of the best bosses in a video game as well...walking out into the expanse of Hyrule Field is what Video gaming is all about

Zelda-A Link To The Past-played it a dozen times,first had it on my SNES but it wasn't until the GBA version that i finished it,amazing game

Knights of the Old Republic-say what?a Star Wars game that's good?,and further more it's an RPG? and the robot in it is the complete opposite of C-3PO where he hates everyone Humanand calls them meatbag?..sign me up

Silent Hill 2-The dread i had while just playing a video game(just a video game i kept telling myself!!) and walking through the fog was imense(something i am looking forward to again in the upcoming HD collection)

Final Fantasy VII-Wait a minute!..has he?...he has I th...has he?,yeah he's just killed her...what the f...!!!...he's just killed her,i fancied her as well!!!...all those memories especially our date at Gold Saucer,and now she's dead...Hang on,i have a Phoenix Down,that should do it!!...oh,i guess not

Lost Odyssey-Oh man,just when i thought JRPGs were out,they drag me back in,it had everything JRPGs should have great storyline,great bosses and great locations and the battles were a joy to fight

Grand Theft Auto: San Andreas-This one had everything,i loved the town to the forest/wood section to the desert,and it had a great storyline and great voicecast as well,and it had the humour
