How far did you get before that error appeared?

Right in the beginning when it reads the update from usb. It doesn't even get to the installation anymore

Is it fat 32 formatted?

I think I'm thinking along the same lines as fi now, It may require formatting to FAT32 by the computer as your PS3 seems unable to do it.

when i tried to do it it said the volume is too large for fat32.

What storage size is the hard drive?

This tool can format any size of drive to FAT32. Normally the limit is 32GB but the PS3 usually has a higher capacity than that

I have formatted the drive to FAT32, and inserted it into the PS3.

I have started in Safe Mode and selected #5, put the USB with 4.11 and started installing.

It is stuck on 99% , same as before the formatting.

What else can be done ?

Is this an issue with the HDD? maybe something else?

Are you doing a quick format, or a full format, or is it not letting you choose? I'd try redownloading the PS3 update file again, or try another USB stick too as it may be that.

I have used the Verbatim Formatter tool.
It formatted to FAT32, and didn't specify quick or not.

How will downloading the file or changing the USB stick may help?

btw once its stuck on 99% I have to format the drive again I think, since it will go straight into update mode, without letting me go to safe mode first.
