This is basically what I do:
In order to gain Discipline I roam around without taking quests and If I took It was 50 war bugle,3 golem kills, 10 geo saurian quests.Like this.Once I went into Man swallowing falls and killed bunch or saurians, 3 skeleton mages and a Cyclops. I unknowingly took a Altar from the box and stored it in Cassardis. When the real quest for Man Swallowing falls came. I could just find 4 altars. It asked me to buy a duplicate Altar from Black Cat shop in Gran soren. I went there, bought the altar and went back to the man outside Man Swallowing Falls. But could not offer him the 5th altar. I searched online a solution and got nothing related to it. But got a hint that I might have taken the altar earlier. So I ran through all rest house and Cassarsdis was the last I visited and I found that green altar which in the note said Quest Item. Then only I was able to finish the quest. I wasted around 3 to 4 hours on it. So everything is related in here. Can't judge whether this dynamicism(??) is good or bad or good for certain limit for rpg. But It has turned me into a more conscious player. I love this thing though.
